Why hasnt any of his worked?
Its really simple actually, if you zoom out, look at the big pciture, Scrum, Scrum Scaled, not Spofitiy adress the overall Operating Model, the MIssion, Vlaue & Business Models undepriing the Opeating Model and Structure of the Busienss/Org/Enterprise.
Its only affected Teams and Indivduals.
Teams ahve little to do with overall Enptrise Agility. The sceret lay elsewher in the orchestration and leadership.
HR have been so impressed with Scrum Master certiifates, we have a hiole bunch of high cost, fake agile roles in place.
HR have been so improessed by other Scrum Certificates, that they have foailed the busienss in building alinged cpabilities that re missing delibertely from Scum Mthods and Theory, becuase thye dont undertnading its use case, cotraints, lmiations and fat that its a dleiberalry incomplete methods.
Scrum Masters, Masetrs of Scrum, the highest cert/next to trainer/who id ont hold must esteem for either, fails itslef, they have maastered none of it tehoery, process and filled the gaps.

Simplfy with Kanban
Ditch the rituals of Scrum for soething simplier like FiFo Kanban or JTBD.
First in First Out Kanban will vosuaile you work flow siply, effectively to tame any chaos in demands, priotisation and capacity.
Start with a Backlog of all work, any types, issues, risks, anything, its jsut a bucket, a container to collec it in and make it visible.
The have a To Do Column - this si for sorting all he work out into first in, of by using a prioroty system like MoScoW or P1 - 4 ie MIssc cirtical ,loosing meg bucks er hour, per day to we can work round it for now, ubt needs fixing.
WIP Working Process or Progress. This si the clevler part, the "control". Kanban is a pull System, not a push system of work. So the workers decides whenn they can do someting, not the Manager, a push systems eg Scrum or COmmand and Control Managment by Auhority. Each Team Memeber selects the highest priorty work from the tp of the coloun first, over any lower items, a superviosur cna set his up, you can run a "replensihment meeting" to do it by consiesus or use logic. This icreates the order, the calsm, you do as much work as huamnyl possible to "close" the tticket/work items as oyu can before pullng the next item, so you satrt fincihing, stop starting more, unfinshed work.
The Done Clumn - should be as it says on the tin, done, completed work. I wul suggest using a defition of done, thts staes its only done this time round, to solve the issue direct tohand, as it may infact reuire a much larger rice of work to totally slve it. If youcan totlaly solve it firt time round, thats the best options, so do it, but sometimes, we need to kepe things moving, using a next best thing. Just make sure thats very clear to the bsienss.
THis ismple sytem make work visbile, sets priorities, communicates what msot valuable, if its being worked on and when its done.
Do not use SLA/OLAs as this then creates added pressure, let people wait, rome was buit in a day and its not heart or brain surgery, despite all thiere jumping up and down and noise making. Be realstic, be reasonable and Dont be a Dick.
